- Once details are finalized with the conference administration team, download and complete the Exhibitor Agreement.
- Submit the completed form to [email protected] by April 15, 2027.
- New sponsors should also submit a W-9 and supplier information by April 1, 2027. Please review the additional instructions below.
- If you are already working with a conference committee member, please copy [email protected] when submitting.
New Sponsor Requirements
New sponsors and suppliers are those who have never sponsored a University of Minnesota event and the University has never purchased goods or services from.
If you are a new sponsor or supplier, please provide the following to epineuroconf@umn.edu no later than April 1, 2027:
- Purchase Order (PO) address
- Supplier contact name
- PO dispatch email address
- 1099 mailing preference
- Remittance address information (if different from PO address)
- A completed W-9 Form
Once your exhibitor agreement is processed, an invoice will be sent from [email protected]. Payment is due within 30 days of receipt. Please check your spam or junk folders if you do not see the invoice.
